Your health is very important to us. We want to make sure that you are getting the help you need, when you need it. If you are new to the HUSKY Health Program or Charter Oak Health Plan, please use the link below to fill out one of our online surveys. You can complete one for yourself or for your child. We will use the answers to assess whether you or your child may need additional help or support.

To fill out a survey, click the link below. On the next page, where it asks for your PIN, enter your member ID number and click the "logon" button. From there you will be able to complete the survey.
