Opting Out of Sharing Health Information with your PCP on the Secure Provider Portal.
The HUSKY Health and Charter Oak Health Plan Programs give Primary Care Providers (PCPs) computer access to certain information. Your PCP both provides and manages your health care. In addition to your PCP having computer access to certain utilization data (health information) based on his or her own records, unless you tell us otherwise, your PCP will have secure computer access to certain health information about you that HUSKY Health/Charter Oak has from some of your other health providers, such as hospitals, specialists and pharmacies. Your PCP may only use this information for treatment purposes. If you do not want your PCP to have computer access to this information, you will need to complete an Opt Out Request Form and send it back to us.

The HUSKY Health and Charter Oak Health Plan programs are looking for members to join our Member Advisory Workgroup.
The goal of the HUSKY Health program is to help you and your doctors work together to make sure you get the best care possible. HUSKY Health will provide you with a variety of services to help keep you in good health and improve your well-being.
The Charter Oak Health Plan provides health coverage for Connecticut adults of all incomes.
