Medical Prior Authorizations

The Medical Authorization Portal gives providers the ability to electronically submit prior authorization requests for elective and emergency inpatient admissions, durable medical equipment (DME), medical/surgical supplies, rehabilitative therapy services, home health visits, and elective procedures. Through this secure portal, providers can request authorizations, submit clinical information, receive immediate real-time approvals for requests that meet evidence-based medical necessity guidelines, and track the status of requests.

To ensure we continue to provide you with the highest quality of service, we will be upgrading the Medical Authorization Portal beginning on Saturday, 4/6/24 from 11:00 p.m. to Sunday, 4/7/24 7:00 a.m. Eastern. During this maintenance window you will be unable to submit electronic requests for medical prior authorization via the portal. However, you may submit any medical prior authorization requests to Community Health Network of Connecticut, Inc.® (CHNCT) via fax at 203.265.3994. Thank you for your patience.

Medical Prior Authorizations Portal

Medical Authorization Portal support resources for providers who submit authorization requests for inpatient or outpatient care (including home care services) are available below.

The portal allows providers to backdate the prior authorization request up to five (5) calendar days to accommodate for member retroactive enrollment and holidays.

To search for a member, you must use the member’s date of birth (DOB) and HUSKY Health member ID number. If the member does not display, but the member displayed in AEVS, please fax your authorization request and clinical information to 203.265.3994.

Please note: Behavioral Health inpatient authorizations should continue to be directed to CT Behavioral Health Partnership/Carelon Behavorial Health (CT BHP) via phone at 1.877.552.8247. Behavioral Health Home Health authorizations should continue to be directed to CT BHP via ProviderConnect, an online registration application located on the “For Providers” homepage at

  • Chrome 28.x or higher is preferred
  • Screen resolution 1366 x 768 pixels
  • NOTE: IE 10.x or higher and Firefox 22.x or higher may be used
