Glide Path
The Glide Path option is available to Private Practices and Hospital Outpatient Clinics. This option includes financial incentives and assistance from a Clinical Practice Transformation Specialist.
The Joint Commission
The Joint Commission PCMH certification is available to FQHCs and Hospital Outpatient Clinics. This option is a part of our PCMH Accreditation program, which includes assistance from a Clinical Practice Transformation Specialist.
National Committee for Quality Assurance (NCQA)
The NCQA Patient-Centered Medical Home recognition program is the basis for private practices and hospital outpatient clinics to earn incentives for the DSS PCMH program.
