Improving health and healthcare requires a focus on impartiality – fairness in access, treatments, and outcomes. Healthcare providers are in a unique position to work with our HUSKY Health members to increase opportunities for them to be healthier – especially those whose obstacles are greatest.

Help and Support

The Clinical Practice Transformation Specialist (CPTS) team can assist you with accessing actionable data to help you identify and manage your patient population. They can show you how to use analytic tools and gaps in care reports, along with reports from your practice’s EHR. The team also provides education on how to use that data for Quality Improvement.
