The following organizations are accepted by the Department of Social Services (DSS) as Person-Centered Medical Home (PCMH) practices.
In order to qualify as a PCMH practice in the DSS Person-Centered Medical Home program and be eligible for additional financial incentives or enhanced fees, practices must be either an independent private practice or a hospital-based outpatient clinic and recognized by the National Committee for Quality Assurance (NCQA) as a Patient-Centered Medical Home.
Community Health Centers and Federally Qualified Health Centers (FQHC) are eligible to participate in the PCMH Accreditation Program and receive technical assistance. These PCMH accreditation providers receive non-financial support to assist in obtaining or maintaining either NCQA PCMH recognition or Primary Care Medical Home certification for practices accredited by The Joint Commission in Ambulatory Care.
A non-standard site (i.e. school based health centers, mobile vans, homeless shelters) refers to a practice setting that is staffed by one or more primary care providers and is licensed as a separate health care facility by the Department of Public Health through a parent entity and would otherwise not be eligible for the DSS PCMH program.
Please note: NCQA PCMH recognition is considered only for sites that have specifically achieved recognition, unlike The Joint Commission Ambulatory Care Accreditation including PCMH certification, which recognizes the entire organization.

When creating your new account, you’ll receive an email from Proofpoint. Enter the code from the email in the field provided to complete your registration.
