Your Care After the Hospital

If you or a family member have recently been to the hospital or the Emergency Room (ER), it is important to see a Primary Care provider (PCP) within a week. This follow-up visit will help to keep you out of the hospital.

Nurse Help

To help you set up your follow-up visit and to answer any questions you have about your care, one of our nurses will call you at home. Our nurses have helped many people just like you get the care they need after leaving the hospital. Our goal is to help you get back on your feet as soon as possible.

When our nurses call you at home, they can help you:

  • Schedule follow-up appointments and transportation to appointments if you need it
  • Make sense of any instructions from the hospital
  • Understand your medications and share this information with your provider so that everyone is on the same page. If you have any questions or concerns about any of the medication your provider has prescribed for you, please contact your provider immediately
  • Answer questions about your care
  • Get the equipment and medications you need delivered to your home
  • Know when to call your provider if you are having other health issues, pain, or symptoms
  • Understand the medical system
