Social Determinants of Health

Health Equity means that everyone has the same chance to be as healthy as possible. To help make that happen, Social Determinants of Health (SDOH) need to be taken care of. SDOH means the conditions in which people are born, grow, live, work, and age.

Addressing your needs means getting support with:

  • Affording food
  • Paying utility bills
  • Understanding your benefits
  • Finding employment
  • Getting literacy support
  • Getting to and from medical appointments

Community Health Worker Ambassador Program

As a HUSKY Health member, you have the support of the Community Health Worker (CHW) Ambassador Program. CHWs help you find the resources to meet your needs.

Our CHWs will:

  • Advocate for you, and connect you to the correct organizations or people to address your needs
  • Connect you to resources in your community to help you with clothing, shelter, food, childcare, and more
  • Help you find a primary care provider (PCP) and help you schedule your first appointment
  • Help you understand and access your HUSKY Health benefits
  • Listen to your needs and help you create a plan to meet those needs
  • Make sure you can get the services available to you
  • Meet with you in-person in an agreed-upon location in your own community
  • Provide health education in your language and appropriate to your culture
  • Support your healthcare needs so you can be as healthy as possible
  • Teach you how to navigate the healthcare system

Another resource supported by the CHW Ambassador Program is our Peace at Home Library. The library provides parental support through classes with subjects for all ages of children. To view the variety of classes available, visit our Community Resource page.

In addition to meeting with a CHW in person, you can talk with them by phone or at a neighborhood HUB site or at a community event, where you can get Help Understanding Benefits. To find a HUB site or event closest to you, check the schedule here at our Health Events page.
