Welcome to HUSKY Health! We’re here to help you be as healthy as you can be. As a new HUSKY Health member, there's a lot to learn. Please explore our website to learn all about your HUSKY Health benefits and services. Below, we put together a list of things to help you get started. HUSKY A, B, D, LBWe want to make sure you are getting the help you need, when you need it. Please fill out our online questionnaire if you are a new member or an existing member and have not filled it out. You can complete one for yourself or for your child. We will use the answers to assess whether you or your child may need additional help or support.

You will receive your gray Electronic Benefits Transfer (EBT) ConneCT card in the mail from DSS. If you didn’t get one, call DSS at 1.855.626.6632. If your EBT ConneCT card is lost or stolen, you need to cancel it immediately to prevent anyone from accessing your benefits. You can do this by either logging into the client website by clicking here or by calling the EBT Customer Service line at 1.888.328.2666. Follow the instructions/prompts for reporting a lost, stolen or damaged EBT card. DSS recommends changing your PIN once a month, especially right before your benefits are to be added to your card.
You will receive your HUSKY Health ID card in the mail. Your name, ID number, and the name of your HUSKY program (A, B, C, or D) are on your ID card.
You will receive a HUSKY Health Program guide. Read through the HUSKY Health Program guide and keep it in a safe place. This guide will help you understand the benefits and programs offered by the HUSKY Health program. You will find information on how to use your HUSKY benefits, programs we have to help you manage any complex or chronic health conditions, and important phone numbers. You will also find important tear-outs that you can fill out and keep in your wallet or purse.

Download and fill out a Know Your Numbers CardYou will receive an opt-out form in the mail. If you don't want your health information shared with your Primary Care Provider (PCP), specialist, or hospital, you must complete and return the form within 3 weeks of receiving it.
The letter states you’ve been approved for the HUSKY Health program and serves as proof of your HUSKY Health coverage for 30 days from the date of eligibility. If you have questions about your letter, call The Connecticut Department of Social Services (DSS) Benefit Center at 1.855.626.6632.
If your provider or your provider’s answering service is not available, HUSKY Health provides access to Registered Nurses 24/7. Call 1.800.859.9889 and follow the prompts to reach a Registered Nurse. You can speak with a nurse about any health concern you have. When you have the number handy, a nurse will always be ready to help you!
